If your child has had recent Labs, X-rays, CT scans, or MRI's, please bring the results with you.
If your child is being seen for elevated liver enzymes, we will need a copy of the labs from the referring physician. This will prevent delays at your appointments.
We realize that some insurances do not require a referral from your child's primary care physician. However, we do require one on file for us to send reports, know who to call if we need to reach them, and we would like to have the most accurate information in our system.
If an authorization is required for your visit we will need to have it prior to the visit, to see you. Please contact your primary care physician to ensure we have received it.
