New Patient Information

Everything you need to know before your child's first appointment with us.

Important: If your child has had recent Labs, X-rays, CT scans, or MRIs, please bring the results with you. If your child is being seen for elevated liver enzymes, we will need a copy of labs from the referring physician to prevent delays.

What to Bring

  • Your child's insurance card
  • Referral from your primary care physician (if required)
  • Completed new patient forms (downloadable below)
  • Recent lab results, X-rays, CT scans, or MRIs
  • Current medications list with dosages
  • Photo ID for the parent or guardian

Referral & Authorization

We require a referral from your child's primary care physician on file for us to send reports and maintain accurate information.

If an authorization is required, we must have it prior to the appointment. Please contact your primary care physician to ensure we have received it.

Please arrive 15 minutes early to verify registration and update forms.

Typically 30–45 minutes for your first appointment.

Follow-Up Visit

Follow-up visits typically take 15–30 minutes.

Urgent Requests

Urgent cases are usually seen within 2–5 business days.
